- Personalized Primary Care
- Medication Management
- Transitional Care
- Mobile Lab & Diagnostic Imaging
- Client Responsibilities
2.1 Accurate Information: Clients must provide accurate and complete information regarding their health status, medical history, and any other relevant details.
2.2 Cooperation: Clients are expected to cooperate with healthcare providers and follow the prescribed care plans.
2.3 Payment: Clients agree to pay for services rendered per service as per the agreed-upon rates and payment terms.
- Company Responsibilities
3.1 Provision of Services: We will provide healthcare services in accordance with professional standards, applicable laws, and HIPAA regulations.
3.2 Confidentiality: We will maintain the confidentiality of client information as outlined in our Privacy Policy and in compliance with HIPAA.
- Payment Terms
4.1 Billing: Clients will be billed per service rendered.
4.2 Insurance: We accept all U.S.-based insurance companies. Clients are responsible for verifying their coverage and understanding their financial obligations.
4.3 Late Payments: No late payment fees are charged. However, prompt payment is appreciated to ensure uninterrupted service.
- Termination of Services
5.1 By Client: Clients may terminate services with 7 days’ notice.
5.2 By Company: We may terminate services if clients fail to meet their responsibilities or if it is deemed in the best interest of the client.
- Limitation of Liability
6.1 Scope: Our liability is limited to the amount paid for services rendered.
6.2 Exclusions: We are not liable for indirect, incidental, or consequential damages.
- Governing Law
7.1 Jurisdiction: These terms are governed by the laws of the state of Maryland.
